相传真姓出自夏代辅佐禹的真窥。[1]
一说真姓源于慎姓。南宋宋孝宗名为赵眘，「眘」与「慎」同音，为避讳，原姓慎的一支改姓真，南宋官员真德秀即原姓慎。[3]
朝鲜半岛百济王族中也有真氏，后成为当地的姓氏，韩文读作「진」。[1]
